Contact our Senior HelpLine toll-free at 1-800-252-8966, Monday through Friday, 8:30 a.m. to 5 p.m. about the Department's programs and services for older adults and persons with disabilities. For deaf and speech-impaired communication over the telephone, dial Illinois Relay at 711. In most instances, when looking for assistance with specific programs, callers will be referred to a local office in the area they live.

Requests for Information

All record requests of the Illinois Department on Aging (IDoA) must be made in writing and may also require specific forms and support documentation.  Information releases are subject to federal and state laws and Department policy.  Some information may be redacted, or a request denied based on the sensitivity of the information and the governing statutes, rules, or procedures.  The most common types of record requests are:

  1. Authorization/Consent:  Participant, client, or resident requests for confidential program records (see below for links to IDoA and Ombudsman program authorization forms),

  2. Freedom of Information Act (FOIA) for public records, and

  3. Court Order or Subpoena for confidential participant records.

1. Authorization for Release of Information:

Program Participants and their Legal Representatives may request release of the Participant's records by completing an authorization form and submitting it to IDoA.  The email and mailing addresses are on the forms and listed below.     

For Community Care, Benefits Access, and Adult Protective Services Program records, please complete the Authorization for Release of Information and submit it to IDoA. 

Records requested from the State Long-Term Care Ombudsman Program should use the Authorization for Release of Ombudsman Records form for all resident/participant authorizations.

Please note: if the request is made by a Participant's Legal Representative, the request must include proof or support documentation that the requestor is a legal representative.  Substitute forms may be accepted but must include the information requested on the authorization forms and the required support documentation.

Requests may be emailed or delivered to our Springfield office based on the type of request:

Requests for State Long-Term Care Ombudsman Program records may be submitted by email:   AGING.SLTCOProgram@illinois.gov or mailed to: Office of the State Long-Term Care Ombudsman Program, One Natural Resources Way, Suite 100, Springfield, IL 62702.

All other authorizations and requests should be submitted to IDoA by  email: Aging.Subpoenas.Authorizations@illinois.gov or mailed to IDoA, Office of General Counsel, One Natural Resources Way, Suite 100, Springfield, IL  62702.

2. Freedom of Information Act (FOIA): (Public Record Requests)

There are many categories of records that are already publicly available on the Internet that do not require a Freedom of Information Act (FOIA) request.  Please look at our website for the information.  FOIA requests must be made in writing and submitted to the Department's Freedom of Information Officer.  NOTE: If requesting confidential program records, a signed release or court order authorizing such disclosure is required. Refer to the section above for Authorizations.  Subpoenas and court orders are different from Freedom of Information Act requests. Refer to the next section for subpoenas and court orders.

Please include the following information with each FOIA request:

  • Indicate that your request for public records is being made under the Freedom of Information Act (FOIA).

  • Your name. 

  •  Organization, if applicable.

  • Contact information:

    • Mailing address (Required if seeking paper copies);

    • A phone number where you can be contacted for questions; and

    • E-mail address (Required if seeking electronic copies of records).

  • A clear description of the records you are requesting (Required). FOIA is not intended as a medium for answering questions. Questions should be directed to the appropriate Department staff for response.

  • A declaration that a request is for a commercial purpose if that is your intent.

3. Subpoena or Court Order: 

Subpoenas and Orders may be submitted directly to the service provider or the Department on Aging for Community Care (CCP), Adult Protective Services or Benefits Access Program Client/Participant's records.  Subpoena's involving CCP records must include an authorized release of the information (described above) or a suitable HIPAA Order of Protection. Ombudsman resident or participant records require a court order for disclosure when an authorization or consent is not provided.

  • Orders for Long-Term Care Ombudsman Program and Home Care Ombudsman Program records must be submitted in writing to the Office of the State Long-Term Care Ombudsman Program at AGING.SLTCOProgram@illinois.gov.  

  • All Subpoenas and other court orders for records production or other legal matters should be emailed to: Aging.Subpoenas.Authorizations@illinois.gov or mailed to IDoA, Office of General Counsel, One Natural Resources Way, Springfield, IL 62702.
