Apply to be a Provider
The Community Care Program, administered by the Illinois Department on Aging, offers a statewide network of services to persons age 60 and over. These services offer eligible older adults of Illinois the opportunity to remain in their own homes as an alternative to inappropriate or premature institutionalization in a nursing facility.
Illinois certifies providers for services under the Community Care Program. Any willing and qualified agency may participate in this open procurement process.
NOTE: Statewide Provider Management Training
The Illinois Department on Aging will host trainings every year, generally held in April and September. Details will be sent to qualified providers upon application approval by the Department, at which time the provider will be allowed to complete registration for the training.
Adult Day Service
Adult Day Service is designed especially for older adults who want to remain in the community but who cannot be home alone during the day due to a physical, social and/or mental impairment. Adult day service also provides respite for family caregivers, especially those who are employed outside the home, and socialization for isolated older adults. Services offered in adult day service centers include health, medication supervision, personal care and recreational/therapeutic activities. Nutritious lunches and snacks are served, and special diets are provided.
PLEASE NOTE: Non-participating Adult Day Service Centers under the Community Care Programs may apply for the Child and Adult Care Food Program at any time during the year. Federal Settings Requirements - Per federal HCBS Waiver regulations, (CFR, 441.301(5)), settings that are not Home and Community-Based include nursing facilities, an institution for mental diseases; and intermediate care facility for individuals with intellectual disabilities; a hospital, and any other settings that have qualities of an institutional setting. Any setting that is located in a building that is also a publicly or privately operated facility that provides inpatient institutional treatment, or in a building on the grounds of, or immediately adjacent to, a public institution, or any other setting that has the effect of isolating individuals receiving Medicaid HCBS from the broader community of individuals not receiving Medicaid HCBS will be presumed to be a setting that has the qualities of an institution. Mandatory Background Checks: All adult day service providers are mandated to follow the requirements of Public Act 91-0598. No Adult Day Service provider shall knowingly hire any individual or retain an employee in an adult day service who has been convicted of committing or attempting to commit any of the crimes under the Criminal Code of 1961. The provider must initiate, on each applicant's behalf, a Non-Fingerprint Criminal History Check upon a conditional offer of employment.
Automated Medication Dispenser Service
Automated Medication Dispenser (AMD) service provides a portable device that can store and dispense pills and capsules and remind older adults to take their medication at the right time of day.
An Automated Medication Dispenser provider must:
- Be certified by the Illinois Department on Aging and follow all CCP requirements
- Provide accessible, no-cost AMD equipment
- Program participants’ AMD units, either at home or remotely, according to their medication needs
- Provide initial training to participants and their caregiver(s)
- Provide ongoing training as needed
- Provide technical assistance 24 hours a day, 7 days a week
Rates for this service:
- AMD-only installation and training of equipment unit: $50.00
- AMD-only monthly service unit: $40.00
Emergency Home Response Service
Emergency Home Response Service (EHRS) is a 24-hour emergency communication link to assistance outside the home for older adults with documented health and safety needs and mobility limitations. This service is provided by a two-way voice communication system consisting of a base unit and an activation device worn by the client that will automatically link the older adult to a professionally staffed support center.
In-Home Service
In-Home Service provides assistance with household tasks such as cleaning, planning and preparing meals, doing laundry, shopping and running errands. Homemakers also assist clients with personal care tasks, such as dressing, bathing, grooming and following special diets.
