Pre-planning and knowing what resources are available will better equip you to successfully manage the transition and reduce stress levels. If your loved one is moving from a hospital to a nursing home, be sure to review the information and resources on how to choose a nursing home.
If your loved one is moving from a hospital back home, be sure to review the information and resources on in-home care. Durable Medical Equipment (DME), such as hospital beds, wheelchairs, and walkers, may be covered by Medicare or Medicaid. Assistive Technology, including mobility and accessibility aids, may be available through the Illinois Care Connections program and the Illinois Assistive Technology Program.
