Alzheimer's and Dementia Resources
Illinois Department on Aging, in partnership with the Area Agencies on Aging and their providers, provides home and community-based education, support, and resources to persons with dementia and their caregivers.
Automated Medication Dispenser Program
The Medication Management Demonstration (MMDP) seeks to reduce medication error rates and improve outcomes in clients who have historically been challenged by managing multiple medications at home. An in-home assessment will be completed for each client and a certified pharmacist will conduct comprehensive medication reconciliation and review.
Benefit Access Program
Benefits for seniors and persons with disabilities, such as a fee discount on license plates and free rides on fixed-route transits. Eligibility is determined by age, disability, residency and income.
Community Care Program (CCP)
Older adults in Illinois ages 60+ who are eligible for Medicaid receive services through the Community Care Program (CCP). This program is designed to prevent unnecessary institutionalization of older adults who have difficulty with household and personal care tasks. Core services include in-home, adult day and emergency home response.
Nutrition Programs
The Nutrition Program for older adults provides nutritionally balanced meals in congregate meal settings, provides home delivered meals, promotes socialization and the health and well-being of older adults across the state.
Find Assistance for Grandparents Raising Grandchildren
IDoA and the Illinois Task Force on Grandparents Raising Grandchildren work to locate, assist, and promote awareness of grandparent-headed households. The program has established support groups; financial and technical assistance; and referral assistance.
Find Assistance in My Home & Community for Seniors
CCP is a major initiative to prevent the unnecessary institutionalization of older adults in Illinois, ages 60+ who have difficulty with household and personal care tasks. Services include in-home, adult day and emergency home response.
Find the Right Long-Term Care Facility
Persons ages 60+ seeking admission into a nursing home must go through the IDoA screening process. During the process, all persons seeking information are provided with all short and long-term care options, regardless of income or DON score.
Long Term Care Ombudsman
Mandated by the Federal Older Americans Act and the Illinois Act on Aging, the Illinois Long-Term Care Ombudsman Program (LTCOP) is a resident-directedadvocacy program which protects and improves the quality of life for residents in a variety of long-term care settings. Ombudsmen work to resolve problems of individual residents and to bring about changes at the local, state and national levels to improve care. Ombudsman work is directed by the resident.
Older American Services
Older Americans Act (OAA) funded programs serve older adults who reside in neighborhoods throughout Illinois.
Adult Protective Services
APS protects adults ages 60 or older and people with disabilities ages 18-59. To report suspected abuse, exploitation or neglect of an older person, call the statewide, 24-hour Adult Protective Services Hotline: 1-866-800-1409. For deaf and speech-impaired communication over the telephone, dial Illinois Relay at 711. APS is locally coordinated through 38 provider agencies. Caseworkers receive reports, conduct investigations and work with older adults to resolve abusive situations.
Senior Community Service Employment Program
The Senior Community Service Employment Program (SCSEP) assists adults age 55 and older in entering or reentering the job market. This program fosters and promotes part-time and temporary community service opportunities that contribute to the general welfare of the community.
Senior Companion
Senior Companion agencies provide an array of assistive, supportive companionship services to frail seniors by utilizing volunteers, also age 60 or older, who have limited income.
Senior Health Insurance Program (SHIP)
Senior Health Insurance Program (SHIP) is a free statewide health insurance counseling service for Medicare beneficiaries and their caregivers. The helpline can answer questions on Medicare, Medicaid, enrollment, drug plan information, and more.
