​​​The Department on Aging Director is mandated to appoint at least 18 but not more than 25 members to the Illinois Long-Term Care Council.

Utilizing a nomination form, the Department administers a fair and impartial process for recruiting and receiving nominations for members to the Illinois Long-Term Care Council from:

• the State Long-Term Care Ombudsman,*

• the Area Agencies on Aging,*
  

• regional Long-Term Care Ombudsmen,*
  

• provider agencies,* and
  

• other public agencies.*

Recommend someone as a potential member by completing the nomination form. 
*Make a copy for yourself and mail the original as directed on the form.

* The Illinois Act on Aging has mandated that these ag​​encies solicit names and recommend members to the Department for appointment to the Illinois Long-Term ​Care Council.
