Nominate Someone!
The Department on Aging Director is mandated to appoint at least 18 but not more than 25 members to the Illinois Long-Term Care Council.
Utilizing a nomination form, the Department administers a fair and impartial process for recruiting and receiving nominations for members to the Illinois Long-Term Care Council from:
the Area Agencies on Aging,*
provider agencies,* and
other public agencies.*
If you would like to recommend someone as a potential member, complete the Nomination Form.
*Make a copy for yourself and mail the original as directed on the form.
* The Illinois Act on Aging has mandated that these agencies solicit names and recommend members to the Department for appointment to the Illinois Long-Term Care Council.
