Reporting Fraud

Fraud can be perpetrated against individuals, organizations, or programs and should be reported immediately.

If you suspect a Medicaid provider (e.g., Community Care Program (CCP) provider, nursing home, hospital, doctor, etc.) is committing fraud,

If you suspect a Medicaid provider (e.g., Community Care Program (CCP) provider, nursing home, hospital, doctor, etc.) is committing fraud,

If you suspect a recipient of Medicaid, TANF, or childcare benefits is committing Fraud,

If you suspect Medicare fraud,

If you have been a victim of fraud or a financial scam,
