The Medication Management Demonstration (MMDP) seeks to reduce medication error rates and improve outcomes in clients who have historically been challenged by managing multiple medications at home. An in-home assessment will be completed for each client and a certified pharmacist will conduct comprehensive medication reconciliation and review.
Older adults in Illinois ages 60+ who are eligible for Medicaid receive services through the Community Care Program (CCP). This program is designed to prevent unnecessary institutionalization of older adults who have difficulty with household and personal care tasks. Core services include in-home, adult day and emergency home response.
Mandated by the Federal Older Americans Act and the Illinois Act on Aging, the Illinois Long-Term Care Ombudsman Program (LTCOP) is a resident-directedadvocacy program which protects and improves the quality of life for residents in a variety of long-term care settings. Ombudsmen work to resolve problems of individual residents and to bring about changes at the local, state and national levels to improve care. Ombudsman work is directed by the resident.
APS protects adults ages 60 or older and people with disabilities ages 18-59. Suspected abuse can be reported through the APS hotline. APS is locally coordinated through 45 provider agencies; caseworkers receive reports, conduct investigations and work with older adults to resolve abusive situations.
Provides technical assistance, phone support and counseling in order to help Medicare beneficiaries eligible for the Medicare Part D benefit select and enroll in Part D plans and eligible persons (including those under the age of 60) to apply for Medicare Savings Programs (MSP), Social Security Administration’s (SSA) Low-Income Subsidy (LIS) Program, Medicare Part B Prevention and Wellness benefit and other prescription drug assistance program options.
